How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

Please read our Practice Terms before registering with our practice.

Before you register

To Register as a patient with us, we will need you to complete some registration documents. You can do this online or collect a registration form from the surgery.

You can only register with us if you live within the Merton Lodge Surgery boundary.  To find out whether your address falls within this area please see the map via the link below:

see our postcode checker map here

You will need your NHS number – if you do not know this, please contact your previous surgery or check your NHS app (we need this to find & transfer your records in the NHS system). You can also find this on the NHS website here

You will also need a repeat prescription list from previous surgery – you will need to ensure you have at least 1 month’s supply of medication before filling in this form and starting the transfer.

You can register by either:

The Register with a GP Surgery service makes it easy to register with us.

Click on one of the above link and fill in the quick online form to start the process.

You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

If you need urgent help or medical advice before you are registered, you can contact 111 or attend urgent treatment centres at either Louth or Skegness.

Non-English speakers

You can find guidance and advice on how to become a new patient if you’re first language is not English, or if you’re an asylum seeker. You can find out more information about the NHS and how to register and access emergency services on the NHS Website.

Temporary registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
