Our Practice Nurse holds regular clinics to monitor patients with Long Term Conditions. If you suffer from a Long Term Condition you will be called in for a yearly review appointment, you may be requested to have a blood test at least 2 weeks prioir to your review and/or bring a urine sample along to your appointment.
