If you require any vaccinations relating to foreign travel, you will need to submit a travel form 8 weeks prior to departure. A link to the form can be found below or if you would prefer, a printed copy can be collected from Reception.
The form will help our Practice Nurse advise what vaccines may be required for your travel. Once our Practice Nurse has reviewed the form, you will be contacted by one of our team to make an appointment. If there are no available appointments or it is less than six weeks to departure you will need to go through a travel clinic.

Please Note – Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. However, should this be the case then you will be made aware of any charges before an order is placed.
